Manhunt does it too. You can use the severed head as a distraction item.

Well, you might as well consider it one. Start with Blood Money. The beauty of the (good) Hitman games is hiding in plain sight and working out how you want to dispatch your target, and Blood Money has some very creative ways to go about that if you're thorough enough.

Goddamn. Manhunt is amazing. Forgot about that. I like that Cash hangs it from his belt...

Several hours into the game, not sure how far, I think in the second main mission. I appreciate the obvious love this game gave to Thief fans. Looks great, unique style. Love the variability of ways to complete tasks. And I am glad to have a new stealth game to play. But so far I think it has the same "disappointments" that Deus Ex:HR had. The stealth gameplay is great; but many of the special abilities and power-ups are useless, or just not interesting, save a couple of them. The special power leveling trees are short; quickly get less interesting as you use them. If you are playing stealthy, you never need health or weapon enhancements. So it's not very rewarding. It could use a bit more personality too. Yahtzee is right; even though Gordon Freeman was silent, strangely he had more character. Liking the game, not sure if I am loving it though.
Also I think I am taking longer than many other people. I think my many years of Thief experience has helped me get a bit more out of this game, exploring and finding hidden things.

Same with me... I'm playing it 'exactly' how I play Thief, and I'm 27 hours into the game with two or three missions still to go.

I watched it last week, Yahtzee has a really good point. I don't think for a second Corvin would spare everybody just for the sake of getting an achievement. There were lots of times where I thought it would be justifiable in that world for him to off a few dirty people. Especially some of the thugs and the overseers. Why the hell should he spare the life of the torturer? He wouldn't! Not if this played out as a traditional narrative. Even when I was playing the non-lethal route, there were just some people I didn't give a crap about and needed to be killed. It's kind of frustrating that these moral choice type of games keep going this route.
I still loved playing it though.

I'm late to the party, but I just finished the Campbell mission and I'm going for the Golden Cat now.
So far I really love it. Great mood, good characters and well-built gameplay. Not as stealth-focused as Thief, but it's still vastly superior to a large majority of stealth games in that domain. I'm trying Clean Hands/Ghost on Very Hard on my first playthrough with most of the UI deactivated and without using/purchasing a single power/enhancement beside Blink, and I'm having a blast so far. My computer is pretty cheap but the game runs extremely smooth and loading times are almost non-existent.
Also, shooting people is extremely satisfying, especially when you get busted and have some fun before reloading. :D

I've started playing it now, very early on. Great environments. I'm also taking my time to explore areas, and set up a pretty minimal HUD. I didn't catch on at first you have to hold the choke button down to bring someone down, so I was just clicking it once, alerting everybody, and having to kill everybody anyway... Not a very auspicious start, but I don't want to reload so take it as it comes. Happy to see they carried the tradition of the opening combination lock. I'm not too far in yet though. I'll report more after I've played more.
