Not for the current version of SS1 anyways. In the remakes it`ll be totally do-able.

It would require a total rework of the interface though, since the cursor is currently used for both aiming and clicking on HUD buttons.

I kinda enjoyed System Shock 1's "Robocop" style targetting, it was definitely unique in 1994 and today in 2007 it still is.

The UW engine can't do it. It's as simple as that.
Controls seemed natural enough to me, having played the UW games previously.

I'd be thrilled if I could just re-bind the keys. If I could get mousewheel up/down to somehow control looking up/down, that would make the game about 10 times less tedious for me.

You can already look with the mouse, once it has the arrows just hold LMB. It also moves you around but I don't see how that fits with the engine not being capable of having mouse look?

The game has a set of movement commands (turn left, slide right, move forward, etc.) that cause the view camera to move. They are bound to keys. When you press the LMB, I think it just generates the same movement commands, the set of which depends on the location of the cursor on the screen. In effect, it just translates a mouse click into one or more key presses. That's a big difference from mouse look.
If other companies followed id's model of releasing source code to the community once the game becomes obsolete, then maybe we could do something about it. But otherwise, it seems impossible.
