Sneaking without the gem is not too difficult, it's just frustrating. I'll just stick to ironman to make my games more challenging; without insisting that it be made a core mechanic mind you.
Also, I didn't say sound wasn't a stealth mechanic. I said it wasn't the primary stealth mechanic. Which means that it can get away with more uncertainty.
