We've already heard about upgrades for items like the blackjack (!), and such, and Garrett using something (not the Sentient Artifact we know) called The Eye. If he's using Eye-Drops for abilities, there likely will be different types of them. In the trilogy, upgrades never really were traditional upgrades. He never got more power, really, just access to something new, which was kept minimal. It was mostly in the fiction that any differences were really useful, and the rope-to-vine upgrade depended entirely on texturing of the environment, and nothing else. Con's sword's biggest "up"-grade was that it didn't increase visibility when drawn, and not out of magic, but because the metal was dark. The arrow and blackjack upgrades in TDS only meant certain AIs were easier to deal with, but it was never necessary, and never important, and the whole Rust Mites, Elemental Cocoons (what the heck?), Pagan Sapling, Pagan Cornerstones, and Faction Alliance minigames were just for degrading the difficulty level and using excess arrows up.
Oh, and they've already downplayed the scarring on his face. Again, I hope it goes back to a time before he had healing potions at his disposal.

Is it just me, or do those look like glyphs on his iris?
So we pretty much know that his eye is tied to the focus ability, and that gaining it (either the eye or the ability) is tied to the story, and is mystical. His eye apparently gives off blue smoke. You recharge it by gathering poppies (??) during the level. But at the same time you can turn focus off entirely, which makes me wonder how it can be tied to the story in that case.
My guess? Garrett will begin the game with an injured eye, but some encounter with the mystical orb-thing he finds in the Keeper library/brothel heals his eye and bestows mystical Keeper powers on it.
